Muscle Loss on Wegovy: How to Prevent It

If you're on Wegovy for weight management, you've probably heard about muscle loss. Maybe you've noticed your weight dropping but your body doesn't look or feel the way you expected. Maybe you're weaker in the gym. Maybe your arms feel thinner but your midsection hasn't changed as much as you'd hoped.

This is one of the most important conversations that isn't happening enough around Wegovy.

What's happening

Wegovy (semaglutide 2.4mg) suppresses appetite powerfully. That's the point. But many people end up eating significantly less than their body needs to maintain muscle tissue. When you're barely hungry, hitting adequate protein targets feels impossible.

Research has shown that without resistance training and adequate protein intake, up to 40% of the weight lost on GLP-1 medications can be lean mass rather than fat. Because Wegovy is dosed higher than Ozempic, the appetite suppression can be more pronounced, potentially increasing this risk.

Why this matters more than the number on the scale

Muscle isn't just about appearance. It's the engine of your metabolism. Every kilogram of muscle burns more energy at rest than the same amount of fat. When you lose muscle, your metabolic rate drops. Your body burns fewer calories just existing.

This creates a trap. You stop Wegovy. Your appetite returns. But now your metabolism is slower because you've lost muscle. The weight comes back faster, and it comes back as fat, not muscle.

For women in perimenopause and beyond, this is particularly concerning. Muscle mass is already declining naturally with age and hormonal shifts. Accelerating that loss with rapid weight loss on Wegovy can have lasting effects on strength, bone density, balance, and metabolic health.

How to prevent it

Protein first, at every meal

Aim for 1.2 to 1.6 grams of protein per kilogram of body weight daily. When appetite is suppressed, every meal needs to be protein-forward. A yoghurt for lunch isn't enough. Think eggs, fish, chicken, legumes, Greek yoghurt in meaningful portions. When you can't eat much, what you eat matters enormously.

Resistance training two to three times per week

Not cardio. Not walking (though walking is great for other reasons). Progressive resistance training that challenges your muscles. The weights need to get heavier over time. This sends the signal your body needs to hold on to muscle even in a calorie deficit.

Monitor your body composition

Don't rely on scales alone. A DEXA scan shows you exactly how much of what you're losing is fat versus muscle. Your weight might be dropping beautifully while your body fat percentage stays the same — meaning you're losing as much muscle as fat, which is not a health improvement.

Track your strength

If your lifts in the gym are declining or your grip strength is dropping, that's an early warning sign of muscle loss. Pay attention. Strength is one of the most reliable proxies for muscle mass you can monitor without specialist equipment.

The bottom line

Wegovy is an effective weight loss tool. But weight loss and health improvement aren't the same thing. Losing 15 kilograms of which 6 kilograms is muscle is a trade-off that can undermine your metabolic health long-term.

The solution isn't to avoid Wegovy. It's to use it intelligently: with adequate protein, consistent resistance training, and ideally with someone guiding the process who understands what your body needs to come out of this stronger, not just lighter.
